2025 FALL SEASON REGISTRATION IS NOW OPEN!
To register for the 2025 Mountain View High School Water Polo club team, please complete each of the 4 STEPS below. Once completed, a member of the Mountain View Water Polo club team will reach out to formally welcome you to the team and let you know the next steps in the process.
STEP 1: REVIEW DOCUMENTS
REVIEW & APPROVE:
Both the player and their guardian need to read and confirm that they have read the Code of Conduct form.
CONCUSSION FACT SHEET: Both the player and their guardian need to read and review the Concussion Fact Sheet.
SPORTS PHYSICAL: If you are an incoming 9th or 11th grader, you will need to complete a physical before you can join the team for practice. Send completed forms to this email: Mvwaterpolo1@gmail.com
